Moving house / change of address

If you are moving house, please check that your new home is still within our practice area. If this is not the case, you will need to make arrangements to find a new GP.  Please ensure you inform us of any change of address, telephone number name etc. and please inform the North East Family Health Services Agency (0191 275 4230) too.
