Request an Appointment via SystmConnect
If you request an urgent appointment you will need to be available to come to the surgery the same day and possibly at short notice for examination if required. For Appointments with the Practice Nurse, Healthcare and Phlebotomist please ring in the usual way.
SystmConnect forms can be submitted between 7am and 4pm Monday to Friday. This may vary depending on capacity. Requests submitted after 2pm will still be monitored. If not medically urgent, will be responded to by the end of the next working day. If you cannot use online services, you can telephone or visit us and our Patient Services Team will help by taking necessary details, filling the form with you to ensure everyone is treated equally and fairly. We encourage family members or carers to use the form on patients behalf.
